nella comparsa dall'ascite sono l'insufficiente sintesi epatica di
albumina e l'ipertensione portale. L'addome viene disteso dal liquido
ascitico e dal meteorismo intestinale. L'addome appare svasato ai fianchi
(addome batraciano). La cicatrice ombelicali risulta appiattita.
L'ipertensione intra-addominale favorisce la comparsa di ernie. L'edema
scrotale è frequentemente associato all'ascite. Il sollevamento
diaframmatico può causare dispnea. In circa il 5% dei pazienti cirrotici
può essere osservato un versamento pleurico, usualmente a destra, dovuto
a piccoli difetti del diaframma che consentono al liquido ascitico di
essere aspirato nel cavo pleurico, durante gli atti respiratori, per la
pressione negativa intra-toracica. Se il liquido ascitico viene prodotto
in quantità modesta, può essere aspirato completamente nel cavo
pleurico, tanto che un versamento pleurico destro può essere osservato
anche in assenza di ascite clinicamente evidente.

portal venous hypertension. The abdomen is distended with fluid and
by air in the dilated intestines. The fullness is more conspicuous in the
flanks. The umbilicus is flattened. The intra-abdominal hypertension
favours the protrusion of hernias. Scrotal
oedema is frequent. The raised diaphragm may cause dyspnea. In about 5% of cirrhotic patients
a pleural effusion may be found, usually to the right side. It is due to
defects in the diaphragm allowing ascites to pass into the pleural cavity.
Right pleural effusion may be seen in the absence of ascites due to the
negative intrathoracic pressure during breathing, drawing the peritoneal
fluid through the diaphragmatic defects into the pleural cavity. |
