Fig. 2 - Prurito. Il prurito è attribuito all'aumento degli acidi biliari circolanti dovuto alla colestasi e può essere non proporzionato alla severità dell'ittero. I pazienti con prurito intenso possono presentare lesioni cutanee da grattamento su tutta la superficie corporea. Il paziente mostrato in questa immagine, un uomo di 36 anni con infezione da HIV ed epatite cronica da HCV, venne ricoverato presso il nostro ospedale per prurito intenso e lesioni cutanee che facevano sospettare una infestazione da scabbia. Il paziente era subitterico con 4,5 mg/dl di bilirubina sierica e valori di transaminasi lievemente superiori alla norma. Il prurito venne parzialmente controllato mediante la somministrazione di colestiramina.

Fig. 2 - Pruritus. Pruritus is attributed to retained bile acids due to cholestasis and it may be disproportionate to jaundice. Patients complaining of very intense itching may show cutaneous lesions widespread to all surface of the body with erythema and excoriations of the skin due to intense scraping. The patient shown in this picture, a 36 years old man, with HIV-HCV co-infection and chronic hepatitis, was admitted to our hospital for very intense itching with cutaneous lesions miming scabies. He was slightly icteric, with 4.5 mg/dl of serum bilirubin and mildly raised  AST-ALT level. Relief was achieved by the bile salt chelating resin, cholestyramine.
