Fig. 2 - Cachessia in AIDS. Sintomi sistemici come febbre cronica, sudorazione notturna, diarrea, astenia e perdita di peso sono aspetti propri della malattia da HIV in fase avanzata. Una accurata valutazione diagnostica Ŕ necessaria per poter escludere la presenza di infezioni opportunistiche. Studi autoptici hanno dimostrato che la maggior parte dei pazienti deceduti con una diagnosi di "wasting syndrome" erano in realtÓ affetti da patologie opportunistiche non diagnosticate in vita quali micobatteriosi atipica, tubercolosi extrapolmonare, microsporidiosi, malattia sistemica da CMV o linfomi.

Fig.  - Wasting in a patient with AIDS. Unexplained constitutional symptoms as chronic fever, night sweats, diarrhea, fatigue, and  weigth loss are usually findings of advanced HIV disease. Evaluation to identify specific pathogens that would explain the symptoms is essential before wasting syndrome is diagnosed. Autopsic studies found that most patients who died with a diagnosis of wasting syndrome, had misdiagnosed opportunistic infection such as atypical mycobacteriosis, extrapulmonary tuberculosis, microsporidial infection, CMV disease or lymphoma.
